Wegovy for MASH: what ESSENCE established

A practical reading of the ESSENCE liver trial, fibrosis outcomes and the limits of accelerated approval.

In this guide

MASH is more specific than a fatty-liver headline

The relevant FDA indication is adults with noncirrhotic MASH and moderate to advanced fibrosis, consistent with F2–F3. Wegovy received accelerated approval for this use in August 2025. This does not turn any mention of fatty liver, an isolated liver-enzyme result or suspected cirrhosis into an automatic match. Establishing the diagnosis and stage is part of medical care, not a conclusion to draw from a weight-loss questionnaire.[2]

What ESSENCE measured

The reported interim analysis involved the first 800 participants at 72 weeks. MASH resolution without worsening fibrosis occurred in 62.9% with semaglutide and 34.3% with placebo. Fibrosis improvement without worsening MASH occurred in 36.8% and 22.4%. These are two different biopsy-based endpoints. They should not be merged into a claim that roughly two thirds of participants reversed liver scarring.[1]

Read the two outcomes separately

Imagine a chart with inflammation on one axis and scarring on the other. Improvement on one does not logically require improvement on the other. In the fibrosis comparison, subtracting 22.4 from 36.8 gives 14.4 additional participants per 100 meeting that endpoint, using the reported percentages. That is an absolute difference, not a guarantee for an individual. It also cannot be translated into a count of liver transplants prevented; the outcome being counted must stay attached to the number.

What accelerated approval leaves to establish

FDA describes the approval as based on improvement in MASH and fibrosis, with continued approval potentially depending on verification of clinical benefit. The ongoing trial follows longer-term liver outcomes. A tissue finding can be valuable while still leaving questions about progression, complications and durability. When reading a story about an “approved treatment,” keep the approval pathway and the evidence supporting it together.[2] Our guide to reading GLP-1 research explains why endpoint choice changes the meaning of a result.

Bring a diagnosis-and-access worksheet

Before comparing a commercial program, ask the treating clinician to identify the documented liver condition, fibrosis assessment, treatment goal and who will monitor it. For insurance, request the actual plan requirements for the intended indication rather than assuming a weight-management exclusion decides every use. Record the prescription product and any specialist coordination. An affiliate review can compare service terms, but cannot determine whether a liver-disease prescription is appropriate or covered. For preparing the insurer’s questions, use the prior-authorization guide.

Do not substitute a different liver measure

If a follow-up result shows a lower liver-enzyme value, ask the clinician what that result does and does not establish about the original diagnosis and fibrosis stage. Do not assume it duplicates a biopsy endpoint in ESSENCE. A useful record has a column for the test name, date, result and the clinical interpretation provided. This keeps different measures from being treated as interchangeable and gives a future clinician enough context to understand why a treatment plan was chosen.

Questions that go beyond the scale

Ask which liver measurements will be followed and when the care team will reassess benefit or tolerability. Ask how the plan fits other liver care, rather than treating weight change as the only success measure. If two clinicians are involved, identify who receives results and responds to new symptoms. Keep the written follow-up plan alongside the prescription. This makes the trial useful as background for a specific conversation without converting research averages into your personal treatment instructions.
